ONLINE GRADUATION CONFIRMATION FORM INSTRUCTIONS
Please follow the instructions below to ensure that the information listed on your diploma and in the Commencement program is accurate. It is very important to click the Save button after you complete the form before exiting.

The following instructions will assist you in completing the online graduation form:

Full Legal Name:
If your full legal name is incorrect, you must complete a Name Change Form in person at the Baker Building (110 21st Avenue South) or by mailing a name change request to the University Registrar’s Office, PMB 407701, Nashville TN 37240-7701. Except in cases of data entry error, name change forms must be accompanied by legal documentation (i.e. marriage license, birth certificate, or court order) to support the change. International students may supply a copy of their passport.
You may insert accent marks by clicking on the accent mark box.
Print your name in phonetics as it should be pronounced at the Commencement ceremony.
Home City / Home State / Home Country:
Review/update the Home City, Home State, and Home Country that are listed. This information will be listed in the Commencement program. In order to change the home state which is listed, please be sure that the Country is listed as the United States.
Indicate whether you are planning to attend the Commencement ceremony for this degree.
You have some flexibility concerning how your name appears on your diploma. For instance, you may initialize your first or middle name. Confirmation names and nicknames are not permitted. If you would like your name modified, please indicate this in the diploma name field. We will contact you if your request falls outside acceptable guidelines.
Tentative Latin Honors are calculated for undergraduate students based on their current cumulative grade point average.
College Honors / Honor Societies / Departmental Honors:
If your school offers College and/or Departmental Honors, you may use this section to report the College Honors and Departmental Honors that you are hoping to receive. In addition, you may report participation in an Honor Society.
